First Visit

Please arrive 15 minutes early to complete registration. Bring a photo ID, your insurance card, and a list of current medications. New patient intake forms are available online or at check-in.

Insurance

We accept most major insurance plans, including BlueCross BlueShield, UnitedHealthcare, Aetna, Cigna, Medicare, and TRICARE. Contact our office to verify your specific plan before your visit.

Prescriptions

Prescriptions are sent electronically to your preferred pharmacy. For refills, please allow at least 48 business hours. Call us directly if you need a urgent prescription fill between visits.
